Patient-Centered Medical Home is a new way of practicing medicine. At Winter Park Family Health Center, your physician works with a team of other health care professionals and helps you and your family members actively participate in your own health. As your providers, we ensure that you receive easily accessible, continuous and coordinated care for all your medical needs, especially between medical visits.
The Patient Satisfaction Survey is a short, easy questionnaire that provides us with information and insight on our patients' view of the services we provide. Our goal is to use survey results to design and track quality improvement over time, as well using them to help us identify ways of improving the practice.
We are a private Family Medicine group practice, established in 1986, dedicated to serving our patients with the highest quality care in a professional and compassionate manner. We treat patients from newborns to the elderly and provide state of the art care for acute illness, chronic medical problems as well as minor outpatient surgical procedures.
